If your doctor gives you a treatment plan, it’s important to follow it. Your health could be at risk if you don’t. But if something is making this difficult — like the cost of medications, lack of transportation, not being able to find the right foods, or not understanding your treatment plan – an Optum Care Manager can help you.

A Care Manager is a registered nurse (RN) or licensed practical nurse (LPN) who meets with you or calls you and works with you to find solutions to any issue that may prevent you from following your treatment plan. A Care Manager works one-on-one with you to help you learn about your medical condition and what you can do to improve it. He or she will review with you the signs and symptoms to watch out for, so you can help keep your condition under control and help prevent hospital stays. Your Care Manager is part of your Optum team that focuses on giving you the personalized care you deserve.
