Advance directives

As the name implies, an advance directive makes your wishes for medical and other treatment known in writing, in advance, in case you are ever in a situation where you can't speak for yourself.  Optum encourages you to fill out an advance directive so your medical team can do their best to follow your wishes.

Your Optum doctor’s office will be able to give you a copy of “Five Wishes,” which is a very user-friendly advance directive. Even though it is a legal document, the Five Wishes is easy to understand and fill out. It covers not just your wishes for medical care but also for personal treatment. Please see a sample of the Five Wishes and ask your doctor’s office for a free copy in your language.

Be sure to give a copy of your completed Five Wishes back to your doctor’s office so they will have it on file if they need to use it.
