Shangeetha Balakumar,
M.D. .
Dr. Balakumar is Board Certified in Family Medicine and Geriatric Medicine. She received her Medical Degree from
University of Sint Eustatius School of Medicine, (Sint Eustatius,
Netherland-Antilles. Dr. Balakumar completed her Family Medicine
Residency at Palmetto Health Richland in Columbia, South Carolina and
her Geriatric Fellowship at University of South Florida in Tampa.

Patient Centered Medical Home. A Patient Centered Medical Home is not a

needs. Whether you need blood drawn for lab tests, x-rays taken,
prescriptions filled, or just a routine visit with your primary care provider,
we have the facilities to accommodate you.
The center is open Monday through Friday from 8 am to 5 pm. The center is located in Fountain Oaks Plaza, which is at the corner of
Dale Mabry and Waters. It is most easily accessed from the Dale Mabry side
of the plaza. Fountain Oaks Primary Care Center can provide for all of
your primary healthcare needs. Whether you need blood drawn for lab tests,
x-rays taken, prescriptions filled, or just a routine visit with your
primary care provider, we have the facilities to accommodate you.
