Colleen Ellis, M.D.

Board Certified in Family Practice
Dr. Ellis is a Florida native. She graduated Summa Cum Laude from the University of Tampa in 1984 and the University of Florida College of Medicine in 1988.

After receiving her M.D. she completed the Family Practice Residency Program at Hunterdon Medical Center in Flemington, NJ. She served as chief resident from 1989 to 1991 and was awarded the 1991 Senior Resident Award given by the New Jersey Academy of Family Physicians.

From 1991-2004 she was a partner in a private practice office in New Jersey and also served as the Hunterdon Hospice Medical Director from 1992-1998.

In 2004, Dr. Ellis returned to St. Petersburg and Joined JSA Medical Group to open the Palmetto office in November.

She is married and has 2 children, Kathleen (age 6) and Conner (age 3). She enjoys cross country and downhill skiing. Her special interests include preventative medicine and women’s health.

·        “High Risk” Disease Management Programs

JSA Medical Group takes an interdisciplinary approach to disease management.  All levels of the health care team – physicians, case managers, nursing staff, administrators and pharmacists are involved in the process.

Our program focuses on all the patient’s diseases along with psychological and social issues that effect positive outcomes.  Most other disease management programs focus only on a single, specific disease.

Once a patient has been identified as a “high risk” to be hospitalized or have undesired outcomes, they are enrolled in our high risk program.  Each month (or more if needed) the team meets to develop a plan of care with the ultimate goal to keep the patient out of the hospital.  A typical plan consists of:

1.      Patient education on medication compliance, disease symptoms and when to contact physician

2.      Weekly phone calls to patient to assess current status

3.      Pharmaceutical management

4.      Necessary specialist referrals

5.      Community resources

6.      Daily weight monitoring

7.      Follow up meetings to discuss patient compliance with the patient care plan

·        Patient Education and Group Appointments

JSA utilizes the Group Appointment model to assist in disease management.  For example, a group of 10-20 patients diagnosed with diabetes has a monthly 2 hour meeting with their physician.  The physician provides patient education, performs individual assessments and provides opportunities for patients to discuss other disease related issues within the group.  Each patient is provided with a binder that tracks test results and compliance with preventive services.

·        Pharmacies

Our pharmacies, who operate as AmeriDrug, are conveniently located either within/next to/or near JSA Medical Group locations.  

·        Open Access Appointments

JSA has instituted an initiative called “Open Access” periods.  Open access periods are specific times during the day where no appointments are scheduled, thereby allowing patients requiring immediate care to be seen.  The purpose of the system is to minimize unnecessary visits to the emergency room, as well as unnecessary hospitalizations.

·        Case Management

Our case managers partner with our physicians to ensure our patients are seen on a regular basis, medications are being taken as prescribed and that diagnosed conditions are being addressed. 

By becoming involved in the care of our patients, case managers attempt to prevent conditions from becoming more serious, thereby improving the quality of life of our patients and reducing hospitalizations.
